I hereby signify my willingness for the above mentioned player to play with the club and accept and abide by the constitution and rules of the Harbord United JRLFC Inc, the Manly Warringah DJRLF, the NSWRL and the ARL. I also give permission for the Club to use any team or action photographs featuring the above player for the purpose of promoting the Club in its newsletter, website or advertising. I agree to receive any form of marketing material from the sponsors of Harbord United Junior Rugby League Football Club Inc.
